Yes
If so, how do you do it? Workout log, software program, etc.? Why do you do it this way?
I use "iFitness" on my iPhone. It's an easy to use program which keeps track of my workouts and compares them for me. Good source of motivation!

I never use to, which lead to me always quitting after a week. I used to be "scared" of how people would react if they knew I was working out. What tips would you give to others to help them stay motivated over the long-term?
Workout with others, and make sure people know what you are doing. The support of others is priceless in accomplishing your goals.
