Have you ever had a lifting injury?
Yes
What type of injury was it? How long were you injured? How did you recover?
I injured my rotator cuff using a bench press machine. I recovered by doing a wide range of shoulder rehab exercises.

What is the most common reason that somebody gets injured while lifting?
I don't know what the statistics are, but attempting way more weight than you can handle is probably up there.
