This is pretty much the routine I do with slight variations due to the equipment in my gym.

I'm still working on getting an actual diet plan. I tend to get bored real quick eating the same food every day so I switch things up a lot. I do my best though with eating as many fresh green veggies and lean turkey/chicken.

I use Mens One A Day multi-vitamin.

I use Syntha 6 as my protein.

I use Jack3d as my pre-workout.

I am about to start taking Animal M-Stack.

I use ON Creatine Monohydrate.

After my GF broke up with me dating for two years my motivation became simple.

To take care of myself. To eat and live happy on my own terms. Life is a gift and if you want to really enjoy it, you should take care of what you have to allow yourself the maximum time possible on this earth.
