What is the most common reason that somebody gets injured while lifting?

Lack of Knowledge and breaking form, thats how I got hurt.

What is the most dangerous weightlifting exercise? Why?

Any olympic style power lift cause form and technique have to be perfect and IF you make an error than you get hurt.
