What is the most common reason that somebody gets injured while lifting?
Poor form with too much weight.

Yes
What type of injury was it? How long were you injured? How did you recover?
Many. Dislocated right shoulder, torn ligaments in the left shoulder, torn muscle in my upper back, dislocated hip 3 times. Those are the major ones, others come and go like forearm issues and such.
