What is the most common reason that somebody gets injured while lifting?
Bad form and over reaching. Lack of recovery is probably in there, and just plain ignoring what yopur nbody is telling you.

At least for me, anything behind the neck is bad mojo, and I think most people should be cautious with those kind of movements.
In general, probably the bench press, because so many people do it and let their ego get away from them,so it tends to wreck a lot of shoulders. Plus, unspotted there's a (small) chance it could kill you.Hmmm.
