How many reps per set do you usually do? Why?
8-12, good for strength and size.
How long do you rest between sets on average? Why?
As long as I feel like I need to, nothing more annoying then being unable to finish a set because I didn't rest long enough.
What is the most common reason that somebody gets injured while lifting?

Hi Aphta. Hope you check the answer i posted for Blimp about bf and test/hgh. Hope you recover from those surgeries. My shoulder(and knee) issues were from the smith machine forcing me to lift in a groove out of spec for my structure. Still not completely healed. Best of luck with all your goals.
